Classifer Action Request

This form is used to request that the USPSA staff take corrective action on an Already Submitted classifier. This should be used for corrections such as a member registered in the incorrect class (Open instead of Limited, etc.); incorrect member numbers; or any other erors found after the classifier file has been submitted to USPSA Headquarters.

In order to process a correction, USPSA must received a request via this passworded form, or a signed request from the match director, section coordinator or area director for the classifier in question.

If you specify a change in a competitor score you must include the hit factor. This includes any case where you change a competitor's power factor, or change from a major power factor to production division
Request:

1 - Privacy Notice: This request is considered the equivalent of a classifier score, and will be made available on the USPSA page until it has been processed and entered into the database. We do this so that our members may verify that their requests for corrections have been submitted to USPSA Headquarters.

2 - This is the transaction number assigned by the system at the time the classifier score was submitted to the USPSA web upload system. This field is not required for this form to be processed, but including it will make it easier to promptly and accurately service your request.
